CRAIG A. WARGON, DPM Dr. Craig Wargon is a practicing physician in Podiatric Surgery in the department of Orthopedics at the Kaiser Permanente Santa Clara Medical Center. He is also the physician lead for several clinical software applications for the Technology Group, a regional division of The Permanente Medical Group (TPMG). Dr. Wargon lectures on the subjects of foot diseases and sports related foot and ankle injuries. He has authored scientific articles published in medical journals and numerous chapters for medical textbooks.

PATRICK J. KEARNS, MD
PATRICK J. KEARNS, MD Dr. Pat Kearns is a physician specializing in Internal Medicine, Geriatric Medicine and Nutrition at the Santa Clara Valley Medical Center. He serves as the Director of Chronic Care Management and the Acting Director of the Diabetes Metabolism Clinic.

Urban Miyares
Urban Miyares is a nationally recognized blinded Vietnam veteran with diabetes, an entrepreneur, motivational speaker and educator, mentor, author/writer, inventor and patent holder, a television and film personality, and worldclass athlete!

In 1985, Urban founded the Disabled Businesspersons Association, a national, volunteer-based charity assisting in the rehabilitation and back-to-work of people with disabilities. He is also director of both the Western Region Veterans Business Resource Center and Challenged America, a free learn-to-sail recreational rehabilitation program for kids and adults with disabilities.

Urban was a member of the recordsetting Challenged America 2003 Transpac Team... the first crew of sailors with significant disabilities to race in the 98-year history of the Transpacific ("Transpac") Yacht Race from Los Angeles to Hawaii.

Urban Miyares is recipient of numerous honors and awards for business, athletics, rehabilitation services and community accomplishments, including those by the White House, U.S. Small Business Administration, U.S. Congress, Inc. Magazine, Disabled American Veterans Association (DAV) and Blinded Veterans Association (BVA), Governor of California, San Diego Hall of Champions Museum, and many others.

In 2004, Urban received a kidney transplant and has been able to continue his busy life-style without interruption.

Gary was tasked with the responsibility of designing the future of the residual corporation (The Health Trust) prior to the completion of the sale of the Good Samaritan Health System to a for-profit corporation. The Health Trust is consistently in the 95 percentile of dollars returned to the community against asset value. The Health Trust Model reflects the creativity, vision, and drive to maximize benefits to the community. Through his vision and relentless focus on health issues needing to be addressed, Gary has launched two significant health initiatives in Santa Clara County that will be models for replication in other communities.

Dr. Lopez is the Chief of Medical Services at the Gardner Family Health Network in San Jose. He is highly regarded in the Latino community for his participation with the Santa Clara Diabetes Coalition and Diabetes Society, and for his dedicated leadership of the Community Health Partnership.

PHYLLIS PRECIADO, MD
PHYLLIS PRECIADO, MD
Phyllis Preciado crecio en el Este de San Jose como la hija mayor de una numerosa familia Mexico-Americana que soñaba en convertirse en doctor de medicina. Sus padres no se graduaron de la secundaria y no la pudieron apoyar en su sueño. Sin embargo, ella pudo inscribirse en un programa de enfermería en el colegio local y se convirtió en Enfermera Registrada. Su sueño de covenrtirse en doctor se mantuvo vivo. Varios años mas tarde y después de superar muchos obstáculos, Phyllis Preciado ahora es la Doctora Preciado, mentora y vocera para la comunidad Latina del Valle Central.
